Read Me: Intro, Resources, and Materials
With global average life expectancy at 73 years, age-related ill health is the main driver of healthcare costs, loss of independence, and disability in most countries. Although human biology is complex and there are hundreds of age-related pathologies, the biology of aging can be categorized into a much smaller number of categories and potential treatments. Medically intervening in aspects of aging biology has the potential to increase healthy lifespan in humans and ameliorate, prevent, or reverse age-related health decline and disability.
The umbrella term "longevity" covers a wide range of interests from simple lifestyle advice to hypothetical biomedical rejuvenation to significantly increase healthy lifespan. Beware, as "longevity" is also readily used by quacks and grifters who promote and sell unproven treatments. Because so many subs cover lifestyle (diet, exercise, etc.), it is not allowed in posts here. For those interested in lifestyle, two useful resources are the free substacks of Dr. Eric Topol and Dr. Christin Glorioso, (choose "No thanks" if you don't want to provide your email).
The focus of this sub is biomedical research targeting aspects of the biology of aging, including medical interventions that aim to go through clinical trials and regulatory approval. Continue reading for examples.

Ovarian Aging Isn't Just About Fertility Northwestern Researchers Make the Case for Treating the Ovary as an Upstream Pacemaker for Healthspan
For decades, reproductive biology and mainstream medicine have treated the ovary almost entirely through the lens of reproduction: periods, pregnancy, and menopause. Once menopause hits, the conventional assumption has been that the organ simply becomes defunct or “retired.”

Loss of Cell Identity Drives Human Aging | Epigenetic clocks measure PRC2 low-methylated regions
The post by Eric Topol explains a recent paper that illuminates biological processes that drive epigenetic drift, which epigenetic clocks can measure across mammals. The original paper lays out the importance of PRC2 in this process (paywalled): https://www.nature.com/articles/s41586-026-10955-0

Living conditions in youth were linked to biological aging in early adulthood, Finnish twin study finds
A Finnish twin study looked at whether living conditions and lifestyle from adolescence to early adulthood were linked to biological aging measured from blood at around age 22.
One interesting finding was that higher youth unemployment in the residential area was linked to faster biological aging. Some measures of nearby vegetation were linked to slower aging, although the environmental results were not always straightforward.
This was an observational study, so it can’t show that these conditions caused faster or slower aging. Biological aging was also measured only once, in early adulthood.
I run a small independent research publication called HUMANUMi, and I wrote more about the study here:

Oracle and Buck Institute for Research on Aging Advance Landmark Federal Aging Research Initiative | Buck Institute aims to advance bioinformatics using Oracle Life Sciences Data Intelligence to transform aging research into a predictive, data-driven science
Oracle and the Buck Institute for Research on Aging are collaborating to advance intelligence into age-related disease and support discoveries that can extend the healthy years of life. Buck will leverage Oracle Life Sciences Data Intelligence, which encompasses 122 million+* de-identified longitudinal Oracle Health Real-World Data records and advanced analytics to research biomarkers that aim to enable early detection and targeted intervention for aging-related complications. The research will also be used to pursue qualification for the first FDA-grade "Intrinsic Capacity Score," a vitality score for healthy aging.
Decoding Age-Related Illnesses
The Buck Institute serves as one of the analytics and bioinformatics engines for the Stanford University-led Transforming Health: Reclaiming Intrinsic Vitality for Everyone (THRIVE) consortium of scientists. THRIVE is a component of the PROactive Solutions for Prolonging Resilience (PROSPR) program from the Advanced Research Projects Agency for Health (ARPA-H), an agency within the U.S. Department of Health and Human Services. The THRIVE team's goal is to develop a validated measure of physical and mental capacity designed to help predict long-term health trajectories and enable earlier interventions for age-related decline in a patient's health journey.

Fibroblasts to Regenerate the Thymus and Reboot the Immune System | FibroBiologics
The thymus involutes mostly to fat by age 60, which leads to a decline in immune function, as the thymus trains immune cells to protect one's own cells and attack foreign objects. FibroBiologics aims to use fibroblasts, rather than stem cells, to mitigate thymic disorders and age-related thymic decline.

Can humans radically extend healthy lifespan?
Venki Ramakrishnan is a molecular biologist and Nobel laureate. He recently became interested biomedical research on the biology of aging and wrote a book on the subject. He supports scientific research in the field but is skeptical of the feasibility of dramatically increased lifespan.
