How can I safely transition?

I am stuck in a situation where I will be reliant on transphobic family for a long time.

I could get on HRT, but I'm worried they'll notice physical changes and I'll be in danger.

What should I do?

You're going to have to find the inner fortitude to do one of two things - sublimate the issue until such time as you can strike out on your own and their influence on you is negligible 0R you find the inner fortitude to withstand whatever is going to happen and you go ahead and transition. You should know that the averages are that you will suffer loss more than you will find acceptance. And since you already know what the "lay of the land" is then it shouldn't come as a surprise that transitioning in front of them is going to invite some hostility and toxicity. If you're not strong enough to withstand that then you should not consider transitioning under your current circumstances.

So I'm just screwed, then? It'll never be safe for me, given my disability I'll basically be at their mercy. But I also can't stand not transitioning.

That's a very difficult place to be, and you have my sympathies. That may not help you but you have them anyway. I have known people who, for a variety of reasons, have not been able to transition and have had to make peace with the situation that they are in. I can't actually say that any of them were disabled but their circumstances LED them to a threshold where they had to make that decision. Some people decided to cross the threshold and some people realized that they couldn't. I don't know what your disability is or your mobility is but I do know that you can get some mental health assistance online and do televisits and I urge you to do that. It won't do you any good to let that Fester and eat away at you on an issue that you are not going to be able to act on necessarily. A good therapist will be able to help you develop the tools and coping skills to keep that in perspective and not make it such a dire situation that you can't live with yourself. Please consider it
