The plumbing would be a lot more difficult, the two fixtures use different size drains and venting becomes more difficult with toilets, and alternating the fixtures exacerbates those venting issues on top of the sizing issues. On top of all that drain pipes are pretty large and they want to cram all of that stuff in tight areas.

In short, it would be a nightmare to install this way, very difficult, expensive, and time consuming.

There’s 3 walls with plumbing stub outs in this drawing. That’s high cost too.

Most cost efficient, if you can help it, all fixtures go on one wall which on the other side the other genders restroom would be. Then both bathrooms can share a single common plumbing chase.
