Options: 1. Hemi section and use distal half of the tooth as abutment. It has enough bone support.
- Extraction + implant
I would present both options to patient and let them make the informed decision.

I thought it was a PARL but judging by the downvotes I'll go with the majority. Never too late to learn

The only other option would be hemisection of mesial portion and maintaining distal root for cantilever crown

Maybe you missed the root caries? This thing is way, way beyond gone.
If you’re new you’ll soon learn that it would be impossible to get matrix or anything that far down

Looks like the textbook definition of nonrestorable to me. What could be done otherwise? Hemisection and a bridge?

Root amputation could save the rest

That would be the only way. It’s old school method that docs used before implants, but you really have to ask why though now with implants

Just an alternative. If Patients age, systemic conditions, preferences and financial status matter then it can be considered a potential alternative.

With modern guided implants, why would this be a cheaper alternative? Sounds like a more complex procedure.. obviously NAD

There has to be some biologic douchebag on here that would place a biomimetic onlay with deep margin elevation.

Bulk fill GIC, bleach, post & crown, then extract, graft, implant and don't forget SRP.

this is why GP should learn to do implants. so that the most obvious answer is the one that you wanted anyways. EXT & Implant is much easier and better than trying to save this tooth. that M root decay is non-restorable.

How would one start without ridiculous money spent on courses?
2
u/safeDate4U 11h ago
It’s much more than spending 10k plus on a few courses.

Expose mesial Root, place braces on mesial Root for forced extrusion, then filling to rejoin the two halves
